Overview

Stress Urinary incontinence (SUI) is the involuntary leakage of urine that occurs during an increase in abdominal pressure. SUI is most likely to happen during actions such as sneezing, coughing, laughing, jogging, rapid walking, jumping or heavy lifting. This is caused by weakness in the pelvic Floor and urethra (where urine passes)

Fortunately, there are several highly effective treatments that are available.

Risk Factors of Urinary Incontinence

There are several different types of risk factors contributing to urinary incontinence, such as;

  • Pregnancy and vaginal delivery
  • Pelvic Organ Prolapse
  • Menopause (Incontinence may become worse after menopause as the bladder and urethral tissues weaken due to low estrogen)
  • Age
  • Medications, smoking and caffeine
  • Constipation, obesity, chronic cough, and heavy constant lifting
  • Other Medical Conditions

Lifestyle Modification

  • Modifying water intake
  • Dietary changes - reduction in alcoholic, caffeinated and carbonated beverages
  • Treatment of constipation
  • Regular Exercise
  • Good Toileting habits
  • Modifying the way you lift heavy objects
  • Quitting smoking
  • Weight loss if you are overweight
  • Continence products

Vaginal Estrogen Therpay

  • Incontinence often becomes worse around menopause due to a reduction in the hormone estrogen. Low estrogen can cause either stress or urge incontinence due to vaginal atrophy (weakness) which is due to Genitourinary syndrome of Menopause (GSM)
  • GSM is associated with urinary frequency, bladder irritation and dysuria and can contribute to incontinence.

Pelvic Floor exercises

  • Pelvic Floor exercises (PFE) and bladder retraining has been shown to be more effective in conjunction with a specialist pelvic floor physiotherapist.

Surgery

  • Surgery is often NOT the first line of treatment. Less invasive treatments may improve symptoms and may avoid the need for surgery.
  • The most common surgical options for SUI include a mid-urethral tape procedures. However the best operation to suit your needs will depend on whether there are other additional problems in the pelvic region. This could be if a prolapse is required to be repaired for example.
  • Some patients also require treatment for fibroids or heavy menstrual bleeding.
